Attention to all United Concordia Patients

Effective January 7th 2025, some of our United Concordia patients (Concordia Advantage, and ParNet Advantage) will be considered out of network. We encourage you to review and act upon this notification promptly in order to be prepared for your upcoming appointment in the 2025 calendar year.
Common Plans which will remain in network include:

Advantage Plus 2.0
Concordia Alliance
Elite Plus
Elite Prime
Federal Dental Program Network (FEDVIP)

If you are unsure about your United Concordia 2025 plan details, please contact your employer's human resources department to verify. They can provide information or changes regarding your plan for the upcoming 2025 calendar year.

Despite this arrangement, YOU CAN STILL COME AND INSURANCE STILL PAYS. Please know that all of these employers offer "out of network" benefits and they will still pay towards your services. We greatly value our relationship with those patients affected and want you to know what we're doing to help in this situation. Please call our office so we can personally communicate and develop a plan in moving forward. As a practice, we do not plan on losing any of our existing patients as a result of this transition. We value you and the relationship we have built over time.

For those patients affected and out of network, our office will submit claims to United Concordia but payment for services is due at the time of service corresponding to our office fee schedule. United Concordia will send a reimbursement payment check directly to the patient.

As per the United Concordia customer service representative of our office, this represents only 85 patients of our entire practice. Putting this into perspective, we have over 7000 active patients on our roster. Please see the attached employer list below, noting those effected.
